Aviation, skydiving and space flight demand unparalleled team preparation, and the right decisions in time-critical situations. Many environments present similar challenges, and the same strategies used by flight crews, skydiving teams, and mission control can be highly effective.
Organizational factors in space accidents are explored, and anecdotes from spaceflight and skydiving are used to demonstrate how crew resource management and preparation for time-critical decisions
can be applied to other high-reliability environments.
